Starting October 5th, all staff and students will be required to complete a daily symptom checklist before reporting to the school. Visitors will be required to complete the checklist prior to entering the building or leaving the office.

Based on the responses to the checklist, students and staff will be certified or not certified to come to the school campus. 

To make this process go smoothly LCPS has partnered with Qualtrics to provide an online COVID-19 symptom screening tool. We will be offering 5 different forms of the checklist for students and staff to choose from. These include:

• Email at 6:00 and 7:00 in the morning

• Text (opt-in option) at 6:00

• Paper/Pencil Hardcopy Version

• Generic Link 

• Generic QR Code
