If a disability prevents you from fully performing your duties or using our facilities or having access to our programs, services, or activities, you are encouraged to contact the Leave & Disability Programs office for compliance with the Americans with Disabilities Act (ADA).

To Apply for Accommodations under the Americans with Disabilities Act, please complete the following:

  • Upload all completed forms through our ADA Request Form - Submittal Portal. Please do not email or fax your forms as this will only delay the review and processing of your request.

  • The Leave & Disability Programs Office will discuss your requests with your supervisor and Human Resources & Talent Development.
