To Apply for Accommodations under the Americans with Disabilities Act, please complete the following:
Complete the ADA Employee and Healthcare Provider Forms. This packet includes the Employee Request for Accommodation form as well as the Medical Inquiry form that must be filled out by your medical provider. Please have your medical provider review your Employee Job Descriptions with you. For a copy of your Employee Job Description, please visit https://www.lcps.org/o/hr/page/job-descriptions.
Upload all completed forms through our ADA Request Form - Submittal Portal. This is the preferred way to ensure your paperwork is processed in a timely manner.
The Leave & Disability Programs Office will discuss your requests with your Supervisor and Human Resources & Talent Development.
If American Sign Language (ASL) services are needed, please reach out to your Administrator or designated ASL site coordinator and have them complete the following:
Complete the ASL Request Form - Submittal Portal. Please submit well in advance of the event requiring ASL services so that there is sufficient time to secure an interpreter.
If notice is less than 48 hours before the event, we ask that your Administrator or designated ASL site coordinator call Leave & Disability at 571-252-1690.

The ADA requires employers to provide reasonable accommodations for employees’ sincerely held religious beliefs, unless doing so would cause undue hardship on the operation of the business.
