All important forms needed for Experiential Learning can be found here:
Internship Application: This needs to be completed by the student if they are going to participate in an internship in which they will attend during school hours. All appropriate signatures must be obtained and a site visit by the Placement Coordinator must be conducted before the students can begin the internship.
Internship Tracking Log: This document must be submitted bi-weekly by students participating in an internship in lieu of coming to school. This must be sent via email to the Placement Coordinator and the instructor of the program.
Mentorship Guidelines for the MENTOR: Information regarding what is involved in becoming a mentor for students
Mentorship Guidelines for the PROTEGE: Information regarding the Protege's responsibilities in the mentorship process.
Mentor Paperwork: This paperwork needs to be completed and signed by all parties before the Mentorship can begin.
Shadowing Paperwork: If students are participating in a shadowing experience that was arranged through the Student Services office, the student must complete this information before attending.
Work Permits: For all students ages 14 and 15 who would like to obtain a work permit, please go to the following website and follow the instructions from the Department of Labor regarding work permits.
