Required Documentation

• Birth Certificate

• Proof of Residence (Deed, Lease or Mortgage Statement)

• Parent Photo ID

• Physical dated on/after 8/22/23, a TB screening dated on/after 12/2/23 and immunization records must be provided prior to the first day of school (health form can be found here)
