Student Withdrawal: Complete Withdrawal Form
The withdrawing student's parent/guardian MUST notify Ms. Suzanne Wyker in the School Counseling Office of the student's impending departure and complete a withdrawal form. The form can be obtained from the School Counseling Office or by downloading it here. The student needs 2 days to get the appropriate signatures needed.
