Clinic Reminders
If your child has asthma, diabetes, seizures, life threatening allergies, or other medical conditions, please pick up forms in the school clinic or download the forms from the student health services website. Your physician will need to complete these forms, and a parent or guardian will need to come into the clinic to drop off the form and the medication. This will allow the school to understand and address your child’s specific medical needs.
All medications (prescription and over-the-counter) require the correct form to be completed; otherwise, it will not be accepted. If you have questions regarding which forms need to be completed, please Nurse Meghan.
Prescription medication: must be in the original pharmacy bottle with the proper pharmacy label containing the student’s name, medication, dosage, and administration instructions. If you request, the pharmacy will give you an extra bottle and label for liquid or tablets with the proper amount of medicine for school.
Over-the-counter medication must be in the original package with the name of the medicine and instructions. We suggest you purchase a new bottle if you can.
A parent or guardian must deliver any medication(s) (prescription and over-the-counter) with the completed form(s) to the clinic. Students may not transport any medicine to or from school. Parents are responsible for picking up any unused medication.
Students who have fevers should be kept at home and should be fever free for 24 hours before returning to school.
Rising grade 7 students will require updated immunizations. Please refer to the Required Immunizations for Grade 7 Students section listed on LCPS' Immunization Requirements page.
